When you choose to work with me, we will explore your concerns and history in initial intake appointments, outlining goals for your treatment and charting our course.
Depending on if you are reaching out for individual, group or family therapy, session plans will vary. This is something we will discuss during consultation and in the initial intake appointment.

Once you begin treatment with me, you have a regular and prioritized spot on my calendar and as such I cannot and will not take on additional clients to make up for times when you are not able to attend.
If you are unable to meet and reach out 72 hours in advance, we will reschedule your appointment at another point in the same week. If you are unable to meet and reach out with less notice, the full fee applies.

If your insurance offers a reimbursement option for out of network providers, I’m happy to provide a monthly super bill. It is important to confirm with your insurance provider that this is an option and what coverage they provide.
Full payment is due at the time of service. If you choose to seek reimbursement from insurance I cannot make guarantees on their process.
For couples therapy: I do not diagnose individuals when I see them for couple therapy. As such it is important to ask your insurance if they will cover treatment for the diagnosis of Z63.0 relationship distress and the service code of 90847, family therapy with the patient present. Call the number on the back of your insurance card to find out.
For individual therapy: Be prepared to pay for the first few sessions out of pocket. Typically with insurance, therapy will only be covered if you meet criteria for a mental health diagnosis. I can not diagnose in the first session as it takes a few sessions to establish a diagnosis. If you do not meet criteria for a DSM diagnosis, all of your therapy will be out of pocket.

My rate is 225 per 50 minute session or 255 for 80 minutes. Your ideal session length will be determined by your specific case.
The number of sessions you may expect to have likewise is very case-specific. I use empirically supported approaches in my work with clients to ensure maximum efficacy and efficiency.
If the investment is giving you pause, I keep a limited number of sliding scale spots on my schedule.

This notice describes how information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
Your Health Record and Confidentiality
I keep a record of the health care services I provide you. You have the right to ask me to see and copy that record. You may also ask me to correct that record. I will not disclose your record to others unless you direct me to do so with written consent or unless the law authorizes or compels me to do so.
Your health record contains personal information about you and your health. State and Federal law protects the confidentiality of this information. Protected Health Information (PHI) is information about you, including demographic information, that may identify you and that relates to your past, present, or future physical and mental health, or condition, and related health care services.

My Responsibilities and This Notice
I am required by law to maintain the privacy of your PHI and to provide you with notice of my legal duties and privacy practices with respect to your PHI.
I am required to abide by the terms of this Notice of Privacy Practices.
I reserve the right to change the terms of this Notice at any time. Any new Notice will be effective for all PHI that I maintain at that time.
I will make available a revised Notice of Privacy Practices by sending you an electronic copy, sending a copy to you in the mail upon your request, or providing one to you in person.
If you suspect a violation of these legal protections, you may file a report to the appropriate authorities in accordance with Federal and State regulations.
How I May Use and Disclose Your PHI
I am permitted to use and disclose your PHI in the following ways:
1. Uses and Disclosures Without Your Written Authorization
For Treatment: I may use and disclose medical and clinical information about you to provide you with treatment services (e.g., consultation or referral to another provider).
For Payment: I may use and disclose medical information about you so that I can receive payment for the treatment services provided to you (e.g., providing information to a third party payor or a collection agency for unpaid fees).
For Healthcare Operations: I may use and disclose your PHI for certain purposes in connection with the operation of my professional practice, including supervision, quality assurance, or to remind you of your appointments.
Required by Law: State and Federal law permits me to disclose information about you without your authorization in limited situations, such as:
With a court order or other compulsory process.
For healthcare licensure, public health, or law enforcement reports.
To the US Secretary of the Department of Health and Human Services for compliance investigations.
For health oversight activities (audits, investigations).
Regarding abuse or neglect (limited to necessary information for mandated reports).
Regarding deceased clients.
For research, if approved by an Institutional Review Board with a waiver to authorization.
For criminal activity or threats to personal safety.
For essential government functions (e.g., military, intelligence, protective services).
2. Uses and Disclosures With Your Written Authorization
Other Uses: I must obtain written authorization from you for all other uses and disclosures of your PHI.
This includes: uses or disclosures of psychotherapy notes (with exceptions), marketing, and the sale of your PHI.
Revocation: You may revoke such authorizations in writing. Such revocation will not be effective for actions I may have taken in reliance on your authorization.
3. Incidental Use and Disclosure
I may have an incidental use or disclosure of your PHI (e.g., a conversation overheard) as long as I have adopted reasonable safeguards to protect your PHI and the information shared was limited to the minimum necessary.
Your Rights Regarding Your PHI
You have the following rights regarding PHI that I maintain about you. Any requests with respect to these rights must be in writing.
Right of Access to Inspect and Copy: You may inspect and obtain a copy of your PHI that is contained in a "designated record set." I may charge a reasonable, cost-based fee for copying and transmitting.
Right to Amend: You may request, in writing, that I amend your PHI. If I deny your request, you have the right to file a statement of disagreement.
Right to an Accounting of Disclosures: You may request an accounting of disclosures made for up to six years, excluding disclosures made to you or for treatment purposes or made as a result of your authorization.
Right to Request Restrictions: You may ask me not to use or disclose any part of your PHI for treatment, payment, or health care operations, or to family members involved in your care. I am not required to agree to such restrictions. You also have the right to restrict certain disclosures to your health plan if you pay out of pocket in full for the care.
Right to Request Confidential Communication: You have the right to request to receive confidential communications from me by alternative means or at an alternative location (e.g., a different phone number or email). I will accommodate reasonable written requests.
Right to a Copy of this Notice: You have the right to obtain a copy of this notice from me.
Right to Opt Out: You have the right to choose not to receive fundraising communications. I will not contact you for fundraising purposes.
Right to Notice of Breach: You have the right to be notified of any breach of your unsecured PHI.

Complaints
If you believe I have violated your privacy rights, you may file a complaint in writing with me. You also have the right to file a complaint in writing to the Washington Department of Health or to the US Secretary of Health and Human Services. I will not retaliate against you in any way for filing a complaint.

Starting January 1, 2022, federal laws regulating client care have been updated to include the “No Surprises” Act.
Under the law, healthcare providers need to give patients/clients who do not have insurance or who are not using insurance an estimate of the bill for medical items and services called a “Good Faith Estimate” (GFE) explaining how much your medical care will cost.
What the Good Faith Estimate Covers
This Good Faith Estimate shows the costs of items and services that are reasonably expected for your health care needs for an item or service.
The estimate is based on information known at the time the estimate was created.
The Good Faith Estimate does not include any unknown or unexpected costs that may arise during treatment. You could be charged more if complications or special circumstances occur.
Your Right to Dispute the Bill
If you are billed for more than this Good Faith Estimate, you have the right to dispute the bill.
Contact the provider: You may contact the health care provider or facility listed to let them know the billed charges are higher than the Good Faith Estimate.
You can ask them to update the bill to match the Good Faith Estimate, ask to negotiate the bill, or ask if there is financial assistance available.
Start a dispute resolution process: You may also start a dispute resolution process with the U.S. Department of Health and Human Services (HHS).
If you choose to use the dispute resolution process, you must start the dispute process within 120 calendar days (about 4 months) of the date on the original bill.
There is a $25 fee to use the dispute process.
If the agency reviewing your dispute agrees with you, you will have to pay the price on this Good Faith Estimate.
If the agency disagrees with you and agrees with the health care provider or facility, you will have to pay the higher amount.
